Our practice hours are from 8:30am to 5:30pm, Monday through Friday. Appointments can be made by telephoning our practice at .

What should I bring with me when I come for an appointment?

When you come for your appointment remember to bring the following:

• Health Insurance card
• IPA or Medical Group authorization (if applicable)
• Picture ID
• Patient Information
  • Patient Name
  • Date of Birth
  • Address
  • Phone Number
  • Email address
• Patient forms
• X-ray or imaging study (films) including MRI or CT scan (if done)
• List of medications, vitamins, and supplements currently taking including dosages
• Referring physician name
• Body part(s) or joint(s) to be evaluated
